A machine can follow the words and still miss the human

AI systems operate on representations. They receive language, documents, metadata, public signals, retrieved sources, and prior context. They do not automatically possess the unstated history, relationship context, business nuance, ethical boundary, or lived judgment behind those inputs. When meaning depends on information that was never made available, the system has to infer.

That is why a technically correct answer can still be wrong in practice. The facts can be accurate while the identity is flattened. The citation can be real while the source's meaning is distorted. The recommendation can sound reasonable while routing the user toward the wrong category of provider. The prompt can be followed while the human intent is lost.

The gap becomes more expensive as AI moves closer to decisions
